A nursing home was cited for neglect after a resident with dementia was found lying under her wheelchair at the bottom of a flight of stairs in July, according to a report released Tuesday by the state Office of Health Facility Complaints. At Westwood Health Care Center in St. Louis Park, certain residents at risk of wandering are fitted with electronic wristbands that lock exits when they approach them. After a resident was found at the top of a stairwell one day, she was given a wristband. Eight hours later, she was in an area of the facility without the automatic locks when she wheeled away from an activity, fell down seven concrete stairs and probably fractured two ribs.
