Cummins, Jana L. age 32, of Mpls, passed away May 26, 2015. Preceded by grandparents, Daniel and Julie Jurkovich Sr., Louis Axtman Sr., and Irene DeBernardi. Survived by her husband, Tony; parents, Daniel and Carolyn Jurko-vich; sister, Jill Jurkovich; in-laws, Thomas and Barbara Cummins; and many other friends and relatives.
