Kozak, Paula Jo Oct. 29, 1962 - May 1, 2015 of Sarasota, FL, formerly of Minneapolis, MN. Preceded in death by father, Joseph Kozak and brother, Tim Kozak. Survived by son, Anthony Schultz of Los Angeles; mother, Beverly Kozak; sisters, Carleen (Tim) Murphy, Sue (Peyton) Robb, and Michelle (Dan) Graziano; sister-in-law, Jan Kozak; many nieces, nephews and other relatives. She is also survived by many friends who loved and admired her. Memorial Service to be held at 11:00 a.m. on Saturday, May 30, at St. Austin Catholic Church, 4050 Upton Avenue No., Minneapolis, MN 55412. Reception to follow.
