About 3,000 patients of a Hudson chiropractic clinic have been notified that their personal information may have been breached.

Harel Chiropractic Clinic said a contract chiropractor made copies of patient information in November in order to solicit patients. The employee resigned shortly after the breach.

Information that may have been accessed included the patient's name, address, phone number, e-mail address, date of birth, Social Security number and insurance information. No credit card, bank account or financial information was accessed, according to the clinic.

Although the breach didn't include financial information, the clinic said patients could place a fraud alert on their credit report to help prevent an identity thief from using the information to open credit and other accounts. Patients only have to contact one company:

• Equifax at 1-800-525-6285.

• Experian at 1-888-397-3742.

• TransUnion at 1-800-680-7289.

The other two bureaus will be automatically notified and free credit reports will be sent to the patient from all three companies.
